
Before your appointment
Patients can be seen for physical therapy both with and without a referral. However, many insurance
companies require a referral and/or authorization for treatment. It is recommended that you check with
your insurance regarding benefits.
Prepare for your appointment:
- Download and complete forms:
- Patient Registration
- Policies & Procedures
- Medical History
- Bring referral and insurance card.
- Wear or bring loose fitting clothes that allow for movement and expose the area you are seeking
treatment for (IE. tanktops, sports bras or shorts). Athletic shoes may also be needed for use of
gym.
- You can bring imaging (x-rays/MRI) if you had these taken.
First Appointment (Evaluation)
First visits are for evaluation of the source of dysfunction and usually are one hour in length. Depending
on the nature of the evaluation, individuals can be sore afterwards or more painful. In the evaluation,
your physical therapist may provoke your symptoms. On follow-up visits, we work together to resolve
your dysfunction which will be different from the evaluation. Some individuals may not have soreness
from the evaluation.
Follow up appointments last approximately 60 minutes.
